You can download the request here and give it to your pharmacist who will order the patient ID card for you. Orders made by patients directly are unfortunately not possible.

Cannamedical^® excludes misuse by ensuring that the Cannamedical^® patient ID card can only be ordered in conjunction with a pharmacist. Anyone who is not yet registered as a cannabis patient will not receive a patient ID card. The pharmacist’s signature on the Cannamedical^® patient ID card confirms the patient’s authorization.

In order to obtain a Cannamedical^® patient ID card, you must be a legitimate cannabis patient. Either the patient has already received cannabis from the pharmacy or submits a certificate from the doctor or health insurance.

To ensure credibility, the Cannamedical^® patient ID card is personalized, includes pharmacy information, an identification number and is printed with a hologram. The Cannamedical^® patient ID card may not be passed on to others and must be presented alongside a passport.

Yes, the Cannamedical^® patient ID card can be ordered at any time. Continuous improvements are implemented in cooperation with lawyers and the government authorities.

In order to be able to carry your medicine when travelling abroad, you must carry the certificate completed by your doctor in accordance with Article 75 of the Schengen Convention. The following European countries are excluded from this: Croatia, Romania, Great Britain, Bulgaria, Cyprus and Ireland. Before you can start your journey, the certificate must be confirmed by the highest state health authority. This is then valid for 30 days and must be handled separately for each variety. When traveling outside of Europe, you must comply with the regulations of the respective country.

The Cannamedical^® patient ID card is completely free of charge.

The Cannamedical^® card is valid for as long as the patient receives pharmaceutical cannabis. If the cannabis therapy is terminated, the patient is obliged to irrevocably dispose of the Cannamedical^® patient ID card.

In the case of loss, please contact the pharmacy where you ordered the Cannamedical^® patient ID card. They should report the loss to us. Just fill out our replacement request form and we will gladly send you a new one free of charge.
